The Community Health Representatives (CHR) Program will provide outreach, community education, follow-up, and supportive services to the IHS Service Unit population on, or near the Fort Peck Indian Reservation.
The CHR Program will work to improve attitudes, knowledge, and practices to encourage self-help and wellness to the Indian people of the Reservation.
The Community Health Representatives will participate with IHS in the delivery of a total health and medical care service to the Indian people.
